How To Enter

To enter the Dr. Oz and SleepScore Max Sleep Stakes Giveaway, go to SleepScore Lab’s Sweepstakes Website at www.doctoroz.com/page/sleepscore-max-sweepstakes and follow the prompts to provide the following information: first and last name, mailing address, date of birth, phone number, and e-mail address.

Upon completion of the registration, you will receive one Sweepstakes entry. Limit only one entry per person or household during the Sweepstakes Period.

Who Can Enter

The Sweepstakes is open to legal residents of the United States and the District of Columbia (excluding residents of Rhode Island) who are at least 18 years old as of the Sweepstakes Start Date.

When To Enter

The Promotion begins at 11:59 PM ET on October 23, 2017 and ends at 11:59 p.m. ET on November 20, 2017.

How Sweepstakes Winners Are Chosen

On or about November 23, 2017, 3 potential Grand Prize Winners and 15 potential First Prize Winners will be selected from among all eligible entries received in a random drawing. Potential Winners will receive a Winner Announcement e-mail on Thursday, November 23, 2017.

What Are The Sweepstakes Prizes
