truTV and Hollywood Tans Get Rehab Ready Sweepstakes

Eligibility : The truTV and Hollywood Tans Get Rehab Ready Sweepstakes (“Sweepstakes”) is open to all legal residents of the forty-eight (48) continental United States and the District of Columbia, age 21 years or older at the time of entry

Promotion Period : From 12:00:00p.m. Eastern Time (“ET”) on September 8, 2009 until 11:59:59p.m. ET on October 20, 2009 (the “Entry Period”)

How To Enter : Log on to the Sweepstakes website, located at www.getrehabready.com and complete and submit the online entry form

Limit : Limit one (1) entry per person, e-mail address or household per twenty-four (24) hour period.

Prize (s):

One (1) Grand Prize Winner will receive a prize package with an approximate retail value (“ARV”) of Two Thousand Eight Hundred Dollars ($2,800). The prize package will include; (a) a four (4) day/three (3) night trip to Las Vegas, NV, for two (2); (b) two (2) four hundred dollar ($400) round-trip coach-class air transportation vouchers valid for travel in the Continental United States which Winner and his/her guest may use toward the purchase of airline tickets from a major U.S. gateway airport nearest the Grand Prize Winner’s residence to an airport in the Las Vegas, NV area. Vouchers are not redeemable for cash and Winner and his/her guest are responsible for any/all air travel costs that exceed said voucher amount. All rules/restrictions of airline vouchers apply.; (c) one (1) room, double-occupancy standard hotel accommodations at the Hard Rock Hotel in Las Vegas, NV, for three (3) nights; (d) one (1) year of free UV tanning services and one (1) tanning consultation with a Hollywood Tans senior tanologist, for the Grand Prize Winner only, at a Hollywood Tans location within reasonable distance to the Grand Prize Winner’s residence, in the sole discretion of Sponsor. Sponsor will contact chosen location and arrange for said free services. In the event Sponsor determines the Grand Prize Winner does not reside within reasonable distance of any Hollywood Tans locations, this portion of the prize (1 year free tanning services and said consultation) will be substituted with a cash prize of five hundred dollars ($500); and (e) twelve (12) bottles of Hollywood Tans tanning lotion.

Five (5) First Prize Winners will each receive (a) three (3) consecutive months of free UV tanning services and one (1) tanning consultation with a Hollywood Tans senior tanologist, at a Hollywood Tans location within reasonable distance to the Winner’s residence, in the sole discretion of Sponsor. Sponsor will contact chosen location and arrange for said free services. In the event Sponsor determines the Winner does not reside within reasonable distance of any Hollywood Tans locations, this prize (3 months free tanning services and said consultation) will be substituted with a cash prize of one hundred and twenty five dollars ($125). First Prizes have an ARV of Six Hundred Twenty-Five Dollars ($625).

Winners : For a copy of the Official Rules or the Winners List (available on or about November 29, 2009), send a self-addressed, stamped envelope (residents of VT need not include return postage on rules requests) to the following address by January 30, 2010 (Please specify “Official Rules” or “Winners”) – “The truTV and Hollywood Tans Get Rehab Ready Sweepstakes,” c/o truTV Marketing, 600 Third Ave, New York, NY 10016.
